Services

Who Can Receive Our Services?

Our caring staff provides services to children and adults who have an intellectual or physical disability, brain injury, require behavioral health support and the aging community. Most of our services are provided under the Home and Community Based Waiver Program, which includes:

Home Based Habilitation

We also provide services that are funded through the Regions and Private Pay.

B&D Services, Inc., is contracted to provide services under all three Managed Care Organizations that the State of Iowa has contracted with to oversee Medicaid: Amerigroup, Iowa Total Care and Molina HealthCare.

Waiver Services

B&D Services, Inc. is a Home-Based Community Service Provider (HCBS). We provide Waiver services under the following waivers:

Intellectual Disabilities

Through this waiver our agency provides an array of services. These include:

CDAC (Consumer Directed Attendant Care)
Home and Vehicle Modification
IMMT (Interim Medical Monitoring and Treatment)
Respite
Group Respite
SCL, Hourly and Daily

To be eligible for this waiver you must: be an Iowa resident, be a US Citizen, determined to be eligible for Medicaid, and must have a diagnosis of an Intellectual Disability determined by a psychologist or psychiatrist, this would be measured by an IQ of 75 or less.

Brain Injury

Through this Waiver our agency provides the following services:

CDAC (Consumer Directed Attendant Care)
Home and Vehicle Modification
IMMT (Interim medical Monitoring and Treatment)
Respite
Supported Community Living, Hourly and Daily

To meet the requirements of the BI Waiver an individual must: be between the ages of 1 month and 64 years of age, must be an Iowa resident and US citizen, have a confirmed brain injury diagnosis, be eligible for Medicaid, and must be able to live in a home or in a community based setting.

Physical Disability

Under this waiver B&D provides CDAC (Consumer Directed Attendant Care) and Home and Vehicle Modification.

To qualify and individual must: be an Iowa resident, be a US citizen, have a physical disability, between the ages of 18-64, be ineligible for the Intellectual Disabilities Waiver, and be eligible for Medicaid.

Elderly

Under this Waiver B&D Services provides the following services:

CDAC (Consumer Directed Attendant Care)
Chore
Home and Vehicle Modification
Respite

To qualify for the Elderly Waiver a person must: be age 65 or older, must be a resident of Iowa, meet income and resource guidelines, meet level of care, and receive Case Management services that are coordinated a managed care company.

Health and Disabilities

Under this Waiver B&D Services provides the following services:

Home and Vehicle Modification
CDAC (Consumer Directed Attendant Care)
Homemaker
IMMT (Interim Medical Monitoring and Treatment)*
Respite
Group Respite

To qualify for the Waiver an individual must: be under the age of 65, blind or disabled as determined by receiving social security benefits or by the determination of the Bureau of Long Term Care, need a nursing facility or skilled nursing, and the person must also be Medicaid eligible.

*IMMT is provided to children, parents of the child must be working during services or seeking postsecondary education.

Other Services

Habilitation

We provide Habilitation services to individuals with chronic mental illness. A few of the most common diagnoses that we work with are: Anxiety Disorder, Post Traumatic Stress Disorder, Oppositional Defiant Disorder, Mood Disorder, Schizoaffective Disorder, Major Depressive Disorder and Bi-polar.

Habilitation services are provided in the client's home and out in the their community. We work with the client to build skills and work on their goals and wishes. We can provide Habilitation on an hourly basis or daily basis, this depends on the needs of the client.

Region SCL

Region funded services are available to some individuals that may not qualify for Waiver services. We work with these individuals on goals and skill building. We can work in the clients home and/or out in the community. As the name indicates, the region the client lives in is responsible for providing the payment for these services.

We currently provide services in Buchanan, Fayette, Delaware, Bremer and Black Hawk counties.

Private Pay

Private Pay services are available to individuals that may not qualify for other services. We provide Private Pay services to individuals that need help around their house and in the community. Some examples of private pay tasks that we provide include: housekeeping, meal preparation, doctor appointments, assistance with medications, and bathing. Private services are tailored to your specific needs. We have an hourly rate for services and bill for services monthly.
